THE GATHERING OF DOCUMENTS


​We will need the following documents as part of your application.  We can gather and copy these documents during in of our initial meetings.


  • ​Military discharge document
  • ​Copy of current Social Security Award letter
  • Most recent Personal IRS Tax Return for Veteran and Spouse.  If possible, last 3 years
  • ​Marriage license (if applying as a married couple)
  • ​Veteran’s death certificate (if applying as a surviving spouse)
  • ​Federal tax return - 1040 - any of the previous 3 years
  • ​We will have you sign a DD214 request form to allow us to gather discharge information

THE ONGOING PLAN

Once the VA has approved your application, you will receive payment in the form of a check for care once a month.  The number of hours and monthly cost has been pre-approved by the VA.  Our Family Home Care has several options to help our clients cover the costs of care prior to approval (and complete reimbursement) as well as ideas for ongoing payment and management of funds.  Jene will be happy to walk you through these ideas and the associated process.

THE CARE

Once our Benefit Review team has complete their review, they will let us and you know how much care you will be receiving.  Once the application is mailed to the VA, Our Family Home Care will begin providing you care!  Remember, once the VA approved you for this benefit, they are going to reimburse you for all of your care related expenses that have occurred during the 2-3 months of approval waiting time.

THE REVIEW

Our Veterans Benefit Review Team will be calling you within two days of our completing your VA Benefit Initial Assessment Form.  The purpose of this call is to review all of the information submitted and walk you through the benefit pre-qualification.  This call usually takes 20-30 minutes.  Upon completion the Benefit Review Team can typically let you know whether or not you will qualify for the benefit.

THE PLAN OF CARE


Jene, our owner, will present you with all of the specifics and options involved in securing your care prior to the submission of your benefit application.


Care must be in place (in effect creating a financial hardship) in order to secure the benefit.  Our clients like options when it comes to their care so we   have offer two scenarios for our clients.


  • You private pay for the care until the VA processes and approves your application.  This process can take as long as 16 weeks, however we are seeing typical approval times closer to 10 weeks.  Once your application is approved the VA will reimburse you for all of the money spent paying for care to that point.  We will help you set up your plan of care within the parameters allowed by the benefit.
  • If private paying is not an option, and we understand that it might not be, we have another plan that we’d like to share. Jene can walk you through the details of the options that we support.

THE CONSULTATION

Jene, owner of Our Family Home Care, will visit you in your home.  The purpose of the consultation is:


  • Get to know you, your needs and how we can help.

  • Help you complete the VA Benefit Initial Assessment Form.  This complete form will help us determine, within 5%, whether or not you’ll qualify for the benefit.

  • Put you at ease about the process and answer any questions you may have.
